i have piles from last 7 months i don't know it is starting point or last stage when i take medicine i get rest for some days but after some days problem starts kindly advise

I have constipation since childhood. I am suffering from piles but there is no bleeding. i can feel the piles. Mostly it is a 4th stage pile. i don't have bad eating habits. i eat raw salad, seasonal fruits and green vegetables. i even exercise daily. Still i don't understand the exact problem. Could you plz help me with it? thank you.
