Nothing did go wrong with Moyes. I blame board at MU and Ferguson. Both entity should have groomed a replacement under Freguson 2 to 3 years ahead of time and detected suitability of the replacement.

From your comment it is obvious you don't know football. SAF had recommended Moyes t stick with his backroom staff. That would have helped him groom but Moyes wanted his own team and he had his inexperienced staff who are not accustomed into big time winning. Now if someone hits his own leg on the axe-- then you cannot blame MU board or SAF.
