I am aware of a system working in the signalling of Railway system which is called DATA LOGGER. This is equivalent to the Black Box of the flight.
Railway Board is deeply dependent on the signal position and can make out if there is any mistake at signalling personnel.

Reasons for railway disaster, invariably is the Human error , now superceded by Natural calamity like flash floods.

Who allowed the soil under the track to get washed away???

There are PWI posted for all the Tracks for every 25-30 KMS. There is a GANG under the PWI who is paid for being watchful for the Track problems all the 24hours i.e. they work in shift.

If there is anything untoward happening across their territory of watching, they are supposed to inform both the side nearest Railway stations such that any ongoing train on the route can be put to halt, before a disaster takes place.

Lot of would be accidents are avoided this way.

on 19.07.15, when such a track was washed away near Sindhi Railway station between Nagpur & wardha, an alert informer ran to Sindhi rly stn and all next trains were stopped & routed back.

Here in MP case, it is a clear case of complacency and lack of alertness on the part of Gangman or the PWI who did not bother to take cognizance of the heavy rains or the TRACK conditions in that area.

WHO will investigate all these things???
In the name of Flash Floods , this may get washed away.

Second aspect appears to be indifference of railway inspection team to ignore the repair demand of the catchment / backup portion on both side of the Bridge where no proper strengthening of soil, by proper Stone filling, supported with Cementing work is done.
This has resulted in washing away of adjoin soil, thus leaving the tracks floating??

It is running by way of appeasement to common voters and DA linked pension to its employees. Today pension is Rs.35000 crore whereas total receipts from passengers are Rs.30000 crore plus no sizeable increase as per inflation for lower class of fare, even fare of other classes too are set as per vote bank, not as per need. As such hardly any money is left for safety, upgradation or development. Biggest back gear was given recently by Mamta during her tenure.
