Leonardo as Dom has been trained by US military for altering dreams and entering at a point when the mind is still perceiving the scenes. Once you enter the subjects dream you can have a shared dream and thus extract information from him/her. Dom has also learnt how to guard his mind to prevent others from learning his thoughts. However he experiments seeding an idea into his wife's mind that only death can bring her out into the REAL world, and this leads her to commit suicide. As repent for this experiment grows Dom keeps his wife alive in his sub conscious mind and this affects his ability to build plots. His wife wanted him to die with her, but Dom refuses, to force Dom to die, she has already filed a case with an attorney to report harassment by Dom. Dom therefore has to runaway from US to prevent his arrest.
This is just one plot, there are 2-3 parallel plots and they are all a dream within a dream.
The climax keeps you guessing as to whether Dom actually survives and comes back to REAL life.

There are 5 parallel realities. The last scene shows that the highest most parallel reality where they are dreaming with Fischer on the plane from Los ANgeles to Sydney is also a dream. We do not know what is above that.
Just came out of the theatre , This was the most astonishing film I have seen in my lifetime. The ending has raised a curosity of a sequel to this film as Mr. CObb cud still be in level 1 but without doubt this man deserve a bow from the mankind for this film
