Helmet was on but it would take more than a million tries for ball to breach the helmet to cause such a fatal injury. Nobody is to blame and it is as much a part of professional risk that every driver, window cleaner, offshore rig worker or for that matter anybody in any profession faces. Saddest part is all this was captured on a closest zoom cameras and even after long time nobody could imagine that consequence of the injury would be so fatal.
