The form is no longer filled by ball point. The forms needs to be filled online, and you should take a printout only on a laser printer

The size of the Photograph of 2"x 2" or 37mm x 37mm as mentioned by the author is only half correct.

The correct size of the photograph is 2" x 2" or 50mmx50mm

Before publishing such articles of prime importance, pl ensure the accuracy of contents.
